A potential alternative to Perfusion Angiography Imaging (SPY Elite) for for evaluation of flap perfusion

Tissue perfusion monitoring is an important adjunct in flap and free flap monitoring. SPY EliteR imaging is one of the available tools for flap perfusion monitoring. FLIR-1 is an infrared camera created to evaluate energy efficiency. It is a portable camera, smartphone attachment that may be serve as a cost effective adjunct that does not
require injection of dye to monitor flap perfusion. This preliminary data compared
results from SPY and FLIR-1 imaging on two patients: one evaluation of mastectomy flap perfusion (immediate breast reconstruction with tissue expanders) and one free flap (DIEP Flap) for breast reconstruction.

SPY and FLIR-1 were used for imaging. Mastectomy flap was imaged immediately after placement of tissue expander and skin closure. The DIEP flap imaged after microvascular anastomosis. All images were obtained systematically. The FLIR-1 and SPY images were compared. SPY images showed a more quanitative estimate of perfusion. However, the FLIR-1 model app used in those cases did not contain new technologies such as the Spot MeterR which estimates temperatures and may not represent the achievable possibilities of a more advanced model. FLIR-1 may serve as an adjunct tool in monitoring of flaps. The compact FLIR-1 camera easily fits in a pocket. Assuming similar efficacy, FLIR-1 camera may contain benefits including less costly, portable, and more practical than other modalities. Additionally, FLIR-1 camera does not need intravenous dye administration. We believe that our next steps will be trying to translate its use into a functional flap monitoring tool and even substitute SPY Elite.
