Degloving injury of the foot continues to be a challenging condition to treat. Despite current advancements in reconstructive surgery, most of these injuries may still end up with amputations causing physical and emotional impairment. Typical treatment consists of application of skin grafts or reattachment of the avulsed skin flap. Many physicians encounter challenging issues in regards to predicting avulsed flap viability, knowing the extent of injury and making a logical decision in limb salvage procedure. A case of foot degloving injury, treated by serial surgical operations with a dominant aim to salvage the avulsed tissue, is presented. The avulsed skin flap with no identifiable vessels for microsurgical reattachment, was defatted and used as a full thickness skin graft. Negative pressure wound therapy and dermal regenation template were used as adjunct techniques to help obtain good soft tissue coverage. Traditional methods of treatment for degloving injuries and applications of new wound care technologies are discussed. An algorithm of management of degloving injuries is also presented.
