The Use of Biodesign Biologic Mesh in Immediate Two-Stage Tissue Expander Breast Reconstruction, a Retrospective Case-Series

Background: Tissue expander/implant reconstruction with biologic mesh or acellular dermal matrix has become a popular technique for breast reconstruction. There is a significant amount of data regarding the use of acellular dermal matrices in breast reconstruction such as Alloderm (LifeCell Corp., Branchburg, N.J.), however, there is little data regarding the use of Biodesign Biologic Porcine Xenograft (Cook Surgical, Indianapolis, IN) in breast reconstruction.

Methods: A retrospective review was completed of all patients undergoing immediate two-stage tissue expander breast reconstruction using Biodesign performed by the senior author from May 2011 to December 2012. Data was collected in the areas of patient demographics, operative factors and postoperative course. Data points of interest in the postoperative course consisted of: (1) wound infection requiring PO antibiotics, (2) wound infection requiring intravenous antibiotics, (3) hematoma, (4) skin necrosis, (5) need for explantation of the tissue expander.

Results: Twenty-two patients (35 breasts) were included in the series, (bilateral, n_13; unilateral n_9;). Early postoperative complications occurred in 4 patients [5 breasts n_5 (14.2%)]. Infection requiring hospital admission for intravenous antibiotics and explantation of the tissue expander occurred in two patients [3 breasts n_3 (8.6%)]. Mild erythema necessitating a course of PO antibiotics occurred in one patient [1 breast n_1 (2.9%)]. Mastectomy skin flap necrosis occurred in one patient [1 breasts n_1 (2.9%)]. No hematomas were noted.

Conclusions: Biodesign appears to be a useful adjunct to immediate breast reconstruction with tissue expanders and appears to be another safe alternative in immediate two-staged implant based breast reconstruction.
