Preliminary Results from the Autologous Breast Fat Grafting Database

BACKGROUND: Fat grafting is a versatile technique widely used in breast reconstruction. While clinical studies have found no evidence of increased breast cancer recurrence with fat grafting, laboratory findings are inconsistent. The 10-year recurrence rate of breast cancer ranges from 1-5%. By establishing a data base, we aim to study the long-term impact of fat grafting on breast cancer recurrence compared to the baseline and potentially identify risk factors associated with increased recurrence.

METHODS: A retrospective chart review was conducted on patients who received fat grafting to the breast at UPMC. Patient demographic, medical history, and current breast cancer information were recorded. The operating notes were reviewed for fat grafting technique as well as grafted volume. Follow-up visits were reviewed for changes in breast cancer status, radiological outcomes, and complications.

RESULTS: In 192 patients, the mean age was 51.1 years, the mean BMI was 27.1, and the mean follow-up was 12.4 months. 81.1% of patients received mastectomy and 7.3% received partial mastectomy. 43.8% received chemotherapy, 65.6% received hormonal therapy, and 30.2% received radiation. There was only one case of recurrence. It was detected within 1 month of the fat grafting procedure. Interestingly, prior to reconstruction, the subject denied radiation and failed to complete her chemotherapy regiment despite triple-negative phenotype and Ki-67 of 80%.

CONCLUSION: Although preliminary, this data indicates that there has not been a significant increase in cancer recurrence within the first year following fat grafting. We will continue to follow these patients beyond 5 years.
