Overview of Abdominal Wall Plication: A Review of New Techniques and Modifications

Background: Abdominoplasty is one of the top procedures performed today in plastic surgery. However, there is a lack of consensus regarding specific aspects of its approach. Our aim is to provide a concise source document for plastic surgeons to reference when choosing a plication method to use during abdominoplasty.

Methods: A literature search was performed on Pubmed and OVID to obtain an overview of variation plication techniques used during abdominoplasty. The search terms included "abdominoplasty techniques" and "plication in abdominoplasty". Articles discussing mesh and abdominal wall reconstruction were excluded. Any articles that were not available in full text with an English translation were excluded.

Results: Publications describing various plication techniques were summarized here. Each variation addressed common intraoperative and postoperative complications of abdominoplasty. These included decrease in operating time by utilizing single layer versus 2-layer plication, improvements in epigastric bulging with transverse plication and triangular mattress suturing, and decrease in rupture of the plication utilizing longer acting or permanent sutures. Patient postoperative satisfaction was found to improve when utilizing the novel waist narrowing plication procedures Crossed abdominal wall plication, Crossbow, French-Line and H-shape technique.

Conclusion: A single source document is invaluable when considering the time constraints and variations in patient habitus plastic surgeons manage day after day. By aggregating these techniques locally, describing their key aspects, as well as identifying which abdominoplasty complication they specifically address, we hope to provide a readily accessible surgical reference for abdominal wall plication.
