Comparison of four BMP-based therapies for large bone defect healing in dogs

Objective: The aim of this study was to compare the efficacy of rhBMP-2-mediated bone regeneration therapies with four carriers to heal large canine cranial bone defects.

Methods: Midline 3.5 x 3.5-cm cranial defects were created in twenty-three 14-week-old beagles. The animals were divided into five groups. Group A (n=3) served as a surgical control. Group B (n=5) received 0.2mg/ml rhBMP-2 in an Absorbable Collagen Sponge (ACS) carrier. Group C (n=5) received 0.4mg/ml rhBMP-2 in a Compression Resistant Matrix carrier. Group D (n=5) received 0.2mg/ml rhBMP-2 in ACS with Corticocancellous chips. Group E (n=5) received 0.2mg/ml rhBMP-2 in ACS with MasterGraft Granules. All animals are CT scanned at 0, 8, 16 and 24 weeks postoperatively. Histological evaluation occurs after animal sacrifice.

Results: Seroma ossification occurred in 18 of the 20 animals (90%) that have reached post-operative week 16 after receiving rhBMP-2 (Group B, 5/5; Group C, 5/5; Group D, 5/5; Group E, 3/5). Ectopic bone is least severe in Group E and most severe in Group C. Average defect radiopacity of subjects receiving rhBMP-2 was 99.93% by analysis of pre and post-operative 3D reconstructions (GE Viewer, Northern Eclipse). The surgical control group healed 14.6%.

Conclusions: The degree of ectopic bone formation varied across rhBMP-2 carriers. All groups treated with rhBMP-2 showed near-total defect filling on histology and CT.
