The presentation demonstrates outcomes of breast reconstruction with free flaps in high body mass index patient population. Patients who lost significant amount of weight before surgery to those who did not are compared.

Listeners will be informed about risks and complications of breast reconstruction in overweight and obese patients. Benefits of weight loss before definitive reconstructive procedure will be discussed.

Surgical complications in overweight patients associated with high BMI are well documented. In this study early postoperative complications and the impact of weight loss in the high BMI population undergoing breast reconstruction using abdominal free flaps were evaluated. Multivariate analysis of effect of surgical BMI on complications revealed that overall flap (p=0.008) and donor (p=0.016) complication rates were significantly higher in the obese group. Analysis of preoperative weight loss did not yield a statistically significant reduction in flap (p=0.5161) or donor (p=0.8034) complication rates. Preoperative weight loss, while beneficial for ease of procedure and quality of life, does not diminish increased risk of complications unless the BMI is reduced to normal limits. BMI during surgery is the most important predictor of complications.
