Patient Preferences and Perceptions Regarding their Gender-Affirming Plastic Surgeon

Purpose: The demand for gender-affirmation surgery continues to increase. Plastic and reconstructive surgery (PRS) patients can often choose their surgeon, and thus may have certain preferences. In this study, we sought to better understand the preferences of transgender patients regarding the professional qualities, gender identity, and LGBTQ+ identity of their plastic surgeon.
Methods: A web-based survey was made available from December 5th, 2022 to January 3rd, 2023 to transgender patients from the senior author's practice. Respondents were asked about their preferences regarding the gender identity and sexual orientation of their plastic surgeon, as well as whether they associated certain attributes to gender or LGBTQ+-identity of their plastic surgeon. Qualitative responses were also collected.
Results: Of 461 patients who received emails, 133 completed the survey (28.9%). Most respondents had no preference in surgeon gender (67.6%), though a vast majority with a preference preferred a female surgeon (88.4%). Over half (52.6%) preferred a surgeon who identifies as transgender. A majority believed an LGBTQ+-identifying surgeon would be more empathetic (59.7%) and elicit more patient comfort (61.2%). While 64.7% expressed a preference for an LGBTQ+-identifying surgeon, 71.4% indicated that they would feel more comfortable with a LGBTQ+ surgeon.
Conclusion: While most transgender patients had no gender preference, most would feel more comfortable with a LGBTQ+ surgeon. Many expressed that cisgender, heterosexual surgeons who demonstrate cultural competency can provide equal care irrespective of LGBTQ+ identity. PRS should strive to meet the needs of this community by continued efforts to educate and diversify its workforce.
