Patient-Rated Outcomes after Toe Syndactyly Reconstruction

Introduction:
Toe syndactyly has an incidence of approximately 1/2000 live births and may lead to psychological distress. There are no prospective reviews of patient-reported outcomes (PROs) after toe syndactyly reconstruction. In this study, we sought to evaluate PROs after toe syndactyly reconstruction to improve shared decision-making.
Methods:
A retrospective review of patients was performed. Inclusion criteria were patients under 18 years-old at Riley Children's Hospital who underwent congenital toe syndactyly reconstruction between 2019-2022. The Patient Reported Outcomes Measurement Information System (PROMIS) pain, social interaction, and depression domains and Lower Extremity Functional Scale were utilized. For PROMIS, a score of 50 represents an age-adjusted median score. For LEFS, higher scores indicate greater function.
Results:
Of 24 eligible patients, 6 completed surveys. Average patient age was 2.3 years at time of surgery. PROMIS pain scores averaged 56.4 ± 2.7, social interaction scores averaged 53.1 ± 4.7, and depression scores averaged 46.3 ± 4.2. LEFS scores averaged 92.3 ± 12.4% normal function.
Conclusion:
Congenital toe syndactyly may cause significant stress for the parents/caregivers, given concerns regarding functional compromise and cosmesis. In this study, we found that patients and families reported pain, social interaction, and depression scores similar to age-matched controls. We also found that lower extremity function was excellent. These data can be used as a resource for decision-making when considering toe syndactyly reconstruction for children.
