Outcomes of Strip Craniectomy Techniques for Sagittal Craniosynostosis at a Single Institution

Background
While multiple minimally invasive techniques to correct sagittal craniosynostosis exist, there is a paucity of literature comparing their outcomes.

Methods
A retrospective review was conducted of infants with sagittal craniosynostosis who underwent strip craniectomy (SC) and extended strip craniectomy (ESC) at Akron Children's Hospital from 2010-2022.

Results
Forty-three infants were identified and 32 met inclusion criteria. There were 13 infants in the SC and 19 in the ESC group. Age at surgery, sex, race, preoperative head circumference, preoperative cranial index (CI), transfusion requirements, hospital and ICU LOS were similar between the two groups. Procedure duration was significantly longer in the ESC group (88.3 vs 65.7 min, p=0.02). Head circumference improved in both groups, but the change was greater in the SC group from pre-op to 1 month post-op (2.9 vs 1.5 cm, p<0.01) and overall (7.7 vs 6.2 cm, p=0.01). Change in CI from preop to 1 month postop was similar between the two groups. The change in CI was greater in the SC group from 1 to 12 months post-op (2.8 vs -1%, p<0.01) and overall was greater in the SC group (11.1 vs 7.2%, p=0.01). Two secondary procedures were performed: 1 cranioplasty (SC group) for persistent cranial defects and 1 posterior cranial vault remodeling (ESC group) for ICP concerns.

Conclusion
Simple strip craniotomy allowed for greater change in head circumference from preop to 1 month and greater improvement in cranial index from 1 to 12 months postoperatively when compared to extended strip with a lower operative time.
