Extracranial marginal mandibular nerve injury is seen as a consequence of many surgical procedures. Some of these include parotid and submandibular tumor resections, extra oral reduction of mandibular fractures and neck dissections, to name a few. Sequelae are both cosmetic and functional with varying degrees of oral incompetence. Many corrections have been offered including nerve repair, nerve grafting and tendon transfers. As seen after Karapandzic repairs, the lower lip has a great capacity to expand. We have found direct resection of the paralyzed portion to be uniformiliy effective . Timing is important since some sprouting and re-innervation will occur from the contralateral side. The problem is discussed and representative examples are given.
